What to Bring to Your First Appointment

To provide you with prompt dental treatment, please be sure to bring the following when you visit our practice for your initial visit:

Insurance Information/Card
Photo Identification
Any medical records pertaining to your visit

Please arrive 10-15 minutes before your scheduled appointment time to fill out necessary paperwork.

New Patient Forms

Please feel free to print and complete the New Patient Forms and bring them with you to your first visit. If you are unable to print this material,
please arrive 10-15 minutes early to complete the paperwork in our office.

Appointments

We are currently accepting new patients. Please call our office to schedule an appointment. Once you make an appointment at our office, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, call us as soon as possible to reschedule. Emergency appointments are also available to treat urgent tooth problems.

We are dedicated to maximizing patient’s insurance benefits. Our front office staff would be happy to assist you with any questions regarding your insurance; we process and file claims for any insurance provider.

Fear of being unable to pay for treatment is the number one reason patients neglect regular dental care. At Kearney Dental Clinic, we never want that to be the case for our patients. We assist individual patients in finding a payment plan that works for them.

Don't have insurance? No problem! We offer Care Credit third party financing, as well as in-house financing options. We make sure that every dollar goes toward your care! Don't let the fear of not having insurance keep you from having the smile that you deserve.
